Products I Actually Recommend
Every item graded by evidence level — clinical, emerging, or traditional. Dr. Najafi explains the science, who it's for, and what studies back it up. This is not a list of affiliate links. This is a clinical framework.
Product Categories
pH Testing
Consumer-grade pH strips are accurate to ±0.2 increments — sufficient to track meaningful oral acidity trends. Ideal oral pH: 6.4–7.0. Below 5.5 is high-risk for erosion and cavities.
- 0.5-increment resolution
- Professional-grade accuracy

Microbiome Testing
Home salivary microbiome testing identifies pathogenic bacterial loads before they manifest as disease. The science is advancing rapidly — these tests give you data that wasn't available even 3 years ago.
"Most patients don't realize the mouth has its own ecosystem. Replenishing beneficial bacteria is foundational — it changes everything downstream."
- PCR-based pathogen detection
- 10 bacterial species tested
- Clinically validated accuracy
- Full report with treatment guidance
- 5 major pathogens covered
- Home collection kit
- Digital report within 5 days
- Affordable relative to clinical labs
Oral Probiotics
Streptococcus salivarius K12 and M18 are the two most clinically studied oral probiotic strains — 50+ peer-reviewed trials. K12 targets halitosis and immune defense; M18 specifically inhibits cavity-causing S. mutans through bacteriocin production.
"S. salivarius K12 and M18 are the most studied oral probiotic strains in dentistry. In my clinical experience, compliance is everything — take them after nighttime brushing."
- S. salivarius K12 + M18 dual strain
- 10+ billion CFU

Salivary Diagnostics
Saliva is the most underutilised diagnostic fluid in medicine. It contains biomarkers for systemic disease, pathogen DNA, hormone levels, and inflammatory markers — all collectable at home without a blood draw.
"Salivary diagnostics are underutilized. The data exists — it's been published. The barrier is that most dentists haven't been trained to act on it."
- At-home collection
- Measures diurnal cortisol pattern
- Lab analysis + digital report
- Includes 4 collection tubes
- Same-day lab processing
- 1-sample morning collection
- Digital results within 48 hours
- CLIA-certified lab
Xerostomia / Dry Mouth
Xerostomia (chronic dry mouth) affects an estimated 30% of adults over 65 and is strongly associated with elevated cavity risk. Saliva is the mouth's primary defense mechanism — without it, enamel demineralizes unchecked.
"Xerostomia isn't just uncomfortable — it's a cavity risk. Saliva is the mouth's primary defense mechanism. When it's absent, we lose that protection 24/7."
- Enzyme-based moisturizing system
- No alcohol, sugar, or SLS
- Multiple formats (gel, spray, rinse)
- Clinically tested
- Adhesive discs stick to gums
- Sustained 4–6 hour release
- Xylitol-infused (caries protection)
- Discreet, no water needed
Tongue Scrapers
Tongue scraping reduces volatile sulfur compounds 3× more effectively than a toothbrush alone. Medical-grade stainless steel is the best material — durable, antimicrobial, easy to sterilize, and it lasts for years.
"The research on tongue scraping is clear: 3× reduction in volatile sulfur compounds versus brushing alone. Patients skip this step at their own peril."
- Medical-grade 316 stainless steel
- Proprietary dual-edge design
- Removes 3× more bacteria than standard
- Protective travel case included
- Ergonomic stainless steel
- Natural brand alignment
- Pairs with Davids toothpaste
- Dishwasher safe
- 100% copper material
- Natural antimicrobial properties (traditional)
- Ayurvedic design tradition
- Available via Amazon
Anti-Inflammatory Oral Care
Chronic oral inflammation is a driver of both local disease (periodontitis) and systemic conditions (cardiovascular disease, diabetes, fertility complications). These products target the inflammation directly.
"Chronic oral inflammation is a two-way street with systemic disease. Addressing it changes outcomes — not just in the mouth, but in the whole body."
- Curcumin + CoQ10 + berberine blend
- Targeted anti-inflammatory action
- 3-capsule daily protocol
- No prescription required
- Triglyceride form (3× bioavailability vs. ethyl ester)
- 1000–2000 mg EPA+DHA per softgel
- Friend of the Sea certified
- No fishy aftertaste
- Cholecalciferol (D3) form
- 2000 IU per softgel
- Liquid drops also available
- K2 co-formulation beneficial
Pregnancy & Fertility
Pregnancy increases gingivitis risk by 30–50% due to hormonal shifts. Poor maternal oral health triples cavity risk in children through vertical bacterial transmission. Xylitol use during preconception reduces transmission risk.
"I tell every patient planning pregnancy: your oral health directly shapes your child's. The bacterial transmission happens at birth. You can interrupt it."

Children's Oral Health
Primary teeth set the trajectory for permanent dentition. The misconception that baby teeth don't need care causes enormous downstream damage. Oral microbiome colonization begins at birth — what establishes there matters for a lifetime.
"Primary teeth matter. The misconception that baby teeth don't need care is responsible for enormous downstream damage. Habits start before age 3."

Diabetes Oral Health
If you have diabetes, your mouth is a diagnostic window. Periodontal disease and blood sugar are in a continuous feedback loop — treating one improves the other. Patients with diabetes are 3× more likely to develop periodontitis.

Heart Health Oral Care
The mouth-heart connection has been documented for decades. Periodontal pathogens have been found in atherosclerotic plaques. Treating periodontal disease reduces systemic inflammation — and that directly benefits cardiovascular outcomes.

Cognitive & Memory
The oral-brain axis is one of the most exciting frontiers in medicine. P. gingivalis has been found in Alzheimer's disease brain tissue and is associated with cognitive decline. The science is preliminary but the implications are significant.

Position Statement
On Fluoride
"Fluoride is the gold standard for cavity prevention backed by decades of science. I recommend fluoride toothpaste for anyone at cavity risk. For those who prefer fluoride-free options, nano-hydroxyapatite is clinically proven as a legitimate alternative — though with a smaller evidence base. The choice depends on your risk profile and personal preference, not safety concerns." — Dr. Neusha Najafi, DDS
80+ years of data. Fluoride at 1000–1500 ppm in toothpaste reduces cavity risk by ~25% vs. placebo. Multiple 2024–2025 peer-reviewed studies confirm safety at recommended doses.
The FDA restricted prescription fluoride supplements for children under 3. This does NOT affect fluoride toothpaste, which remains recommended from tooth eruption onward.
Concerns about neurodevelopmental effects at toothpaste doses are not supported by high-quality evidence. Don't avoid fluoride based on internet fearmongering — base it on your risk profile.
Nano-hydroxyapatite works. A 2024 meta-analysis shows comparable efficacy to 500–1000 ppm fluoride for early-stage lesions in low-risk patients. Smaller evidence base, but legitimate.
